What is IVF?

What is IVF?

  • The abbreviation IVF stands for in vitro fertilisation, which means fertilisation outside the human body.
  • IVF treatment is one of the most effective methods for treating infertility and the most used method in assisted reproduction.
  • This treatment is often recommended for couples who have not been able to conceive after one year of unprotected sexual intercourse.
  • IVF treatment can also help prevent the transmission of genetic abnormalities from parent to child.

The basic principle of IVF treatment involves hormonal stimulation of the ovaries to produce an optimal number of small sacs called follicles. Mature oocytes (female reproductive cells – eggs) are then retrieved from these follicles. In the embryology laboratory, sperm is added to the retrieved eggs, which penetrate the egg either naturally or with the help of special laboratory methods. At GENNET, we use the ICSI method for egg fertilisation, where a selected sperm is injected into the egg using a micro-needle. This method increases the
success rate of treatment and is a standard part of all our treatment packages. The fertilised eggs are subsequently transferred back into the woman's uterus during embryo transfer.

During the initial consultation with our experienced IVF doctor, you will discuss your and your partner's medical history, especially in relation to your
reproductive health. In the case of an in-person consultation at our clinic, the doctor may perform a general gynaecological examination for the female partner.
Based on this information, the doctor will determine the possible treatment. If necessary, further tests may be recommended to determine the diagnosis and suggest the most effective treatment.
In case of an in-person consultation at the clinic, it is possible to purchase medications directly in Prague. Purchasing medications in the Czech Republic is more cost-effective for patients and reduces unnecessary stress. If patients obtain medications in their home country, we will send the prescriptions electronically and recommend where these can be purchased.

For successful treatment, additional examinations may sometimes be needed. The uniqueness of GENNET lies in the comprehensiveness of services. We provide complex care under one roof while working across various medical fields that play a vital role in the treatment of infertility. This ensures that the IVF treatment process at GENNET is quick and efficient.
However, the examinations can also be conducted in your home country. However, the examinations can also be done in your home country. Based on the consultation with our doctor, your coordinator will send you supporting documents to help you obtain the necessary tests.
Your lead clinician at GENNET will then request the results of tests necessary for treatment from you. They may also recommend/request additional examinations that could help increase the success rate of treatment. You can send us the results via email, and you will only need to come to Prague for the final part of the treatment, which lasts approximately a week.

Additional Necessary Examinations Additional Necessary Examinations

After diagnosing the patient, the doctor establishes an individual treatment plan and creates a personalised stimulation protocol for the patient.

  • Hormonal stimulation typically begins in the first few days of the new menstrual cycle (with the onset of menstruation).
  • The ovaries are stimulated using a combination of hormones, which the woman administers according to the stimulation protocol, specifying the exact dosage and timing of medication.
  • Hormonal medications may include follicle-stimulating hormone (FSH), luteinising hormone (LH), and sometimes human chorionic
  • gonadotropin (hCG)
  • The effect of stimulation is then monitored by ultrasound examination. Usually, two ultrasound checks are needed during one cycle of hormonal
stimulation.
  1. You will typically undergo the first ultrasound check-up in your country, and you will send us the results via email.
  2. We recommend having the second ultrasound check-up directly at GENNET. Having this examination performed by our doctor using our
equipment helps us maximise the success of the treatment. If you undergo the second ultrasound with us, we will provide the
necessary preoperative examination for egg retrieval free of charge for you.
  • The purpose of hormonal stimulation is to create the optimal number of follicles from which eggs are obtained and used for subsequent in vitro fertilisation.
  • Once the follicles reach the desired size, the patient administers a final injection to induce ovulation, which is followed by egg retrieval.
  • In case of an in-person consultation at the clinic, it is possible to purchase stimulation medication directly in Prague. Buying the medication in the Czech Republic is more cost-effective and stress-free. If patients obtain medications in their home country, we will send the prescriptions electronically and recommend where these can be purchased.
Hormonal Stimulation Hormonal Stimulation

Approximately 14 days after hormonal stimulation, the follicles reach optimal size for retrieval.

  • The egg retrieval procedure is performed under analgosedation or general anaesthesia, ensuring it is painless and lasts 10 to 15 minutes.
  • Due to the general anaesthesia/analgosedation, the patient must undergo preoperative examination (ECG, blood and urine tests, and an examination by an internist). If the patient undergoes the second ultrasound during hormonal stimulation at our clinic, we provide the preoperative examinations free of charge at our clinic.
  • OPU is conducted under ultrasound guidance using a special thin needle, which the doctor inserts through the vaginal vault to the ovaries.
  • During this short procedure, follicular fluid containing follicles with oocytes (eggs) is aspirated from the ovaries.
  • In the laboratory, oocytes are then removed from their follicular shells, and under a microscope, the embryologist evaluates their maturity and quality.
  • If for a medical reason, the patient's eggs are not suitable for fertilisation, we recommend considering infertility treatment with an egg donor.
  • Egg donation in the Czech Republic is completely anonymous. Patients only receive basic characteristics of the donor.
  • From our extensive database, we select the donor based on the physical characteristics of the couple, including blood type and physical appearance:
    • ethnicity
    • skin color
    • eye and hair color
    • height and weight
Egg Retrieval Procedure Egg Retrieval Procedure

The primary fertility examination for men is a semen analysis (spermogram). Microscopic examination of semen evaluates the number of sperm in the
ejaculate, their motility, the shape of their heads, necks, and tails, and overall sperm viability. Through spermogram, we can determine the potential of sperm ́s ability to naturally fertilise an egg.

  • In assisted reproduction, the partner's sperm is mostly collected on the day of the partner's egg retrieval.
  • If the spermogram results are good, the sperm can be used for fertilisation or frozen for future use.
  • In case of deteriorated spermogram values, we recommend visiting a urologist/andrologist.
  • If sperm is absent in the ejaculate, it can be resolved through surgical sperm retrieval using TESE or MESA methods.
  • Poor spermogram results or inability to obtain viable sperm through microsurgical procedures may lead to the recommendation for infertility
treatment using sperm donor.
  • Donating gametes in the Czech Republic is entirely anonymous. Patients only receive basic characteristics of the donor.
  • From our extensive database, we select a donor based on the physical characteristics of the couple, including blood type and physical appearance:
    • ethnicity
    • skin color
    • eye and hair color
    • height and weight
Sperm Collection Sperm Collection

The eggs obtained during egg retrieval are subsequently combined with sperm in a laboratory dish.
At GENNET, all eggs are routinely fertilised using intracytoplasmic sperm injection (ICSI) method. The egg fertilisation success rate with this method is around 90% and therefore is most commonly utilised. The ICSI method is included in the price of all our treatment packages.
Based on test results or previous treatment cycles, the embryologist/doctor may
recommend selecting sperm via microfluidic sperm sorting (MFSS) method.

Fertilisation Fertilisation

The quality of the embryo is determined by:

  • Egg quality
  • Sperm quality
  • Genetic information of both partners

The more quantitative and qualitative data the embryologist has about the embryos, the better they can assess which embryo is most suitable for transfer.
To determine the embryo quality most accurately our experts take advantage of special laboratory methods:

Preimplantation genetic testing can also help with selecting a suitable embryo for transfer. In this case, the embryo undergoes genetic testing before being
transferred to the uterus.
For such genetic embryo testing, we have two methods available:

All embryos undergo preimplantation genetic testing at the blastocyst stage (approximately 5-day-old embryos after extended cultivation). After the sample
is collected, the embryo is frozen. Processing of the test results takes approximately 30 days. Upon receiving the results, the patient begins preparation for the transfer of a healthy embryo at the beginning of a new cycle. In the case of a frozen embryo transfer, the patient only needs to visit Prague for 2 to 3 days.

Embryo Development Embryo Development

Thanks to the high efficiency of assisted reproduction methods, treatment success, and above all, efforts to minimise risks associated with multiple pregnancies, we transfer only one embryo into the patient's uterus (in exceptional cases, two embryos).
The embryo transfer into the uterus takes place while the patient is fully conscious, and approximately 15 minutes after the procedure, the patient can leave the clinic.
If multiple good-quality embryos are obtained from the treatment cycle, we can preserve these for the future using vitrification (freezing).

Embryo Transfer Embryo Transfer

In a natural menstrual cycle, the luteal phase begins with ovulation and ends with the next menstruation. In an IVF cycle, this period roughly starts with egg retrieval and continues for the following 14 days. During this time, a woman undergoes embryo transfer, and subsequently, the transferred embryo should implant in her uterus.
To increase the chances of successful embryo implantation, hormonal medication such as progesterone supporting the uterine lining may be prescribed for the patient after embryo transfer.

Hormonal Medication After Embryo Transfer Hormonal Medication After Embryo Transfer

Approximately 14 days after embryo transfer, the patient undergoes a blood test to determine the level of hCG hormone. The level of this hormone determines whether the patient is pregnant.

Who is IVF Treatment Suitable for?

What precedes IVF treatment?

During the initial consultation with one of our specialists, your medical history will be thoroughly discussed, all previous examinations or treatments will be reviewed, and options for subsequent treatment will be recommended. Additionally, appropriate supplementary laboratory methods and an initial cost estimate based on the current price list of our services will be proposed.

A comprehensive understanding of your and your partner's current health status will be obtained through a series of complex examinations according to the doctor's indication. These tests aim to clarify your health status and determine any correlations with reproductive disorders. Recommended examinations may vary, but the basic ones that all patients undergo include the following:

Women's Examination

  • Recent gynaecological examination
  • TSH, prolactin
  • Assessment of ovarian reserve - the test determines the level of Anti-Müllerian hormone in the blood and thus the available supply of woman's eggs
  • The doctor may request additional tests if considered necessary:
  • Hormonal profile (LH, FSH, estradiol) - must be done on the 2nd - 4th day of the cycle

Men's Examination

  • Semen analysis - basic fertility examination for men, which assesses the quality and quantity of sperm in the ejaculate

Examination of Both Partners

  • Examination for sexually transmitted diseases (STDs) - according to the laws in the Czech Republic, handling of human reproductive cells is only possible under the condition of negative tests for these diseases.

Success rate

Thanks to the excellent job of our experienced embryologists we are able to achieve more 85% rate of successfully fertilized eggs. This is increasing the chance to pick the best embryo for the transfer. The statistics of the successful treatments at Gennet Clinics are based on the number of clinical pregnancies of our patients.

Frequently Asked Questions

How long does the entire IVF process take?

At GENNET, there are no waiting times to start a stimulation. The patient can start as soon as they have completed all necessary examinations and have obtained the medications. The stimulation itself lasts approximately 2 weeks until egg retrieval, and about another 5 days later, the patient undergoes embryo transfer.

What is the age limit for IVF treatment?

According to the laws in the Czech Republic, we can provide assisted reproduction treatments to heterosexual couples where the woman is younger than 49 years old (48 years and 364 days).

What are the benefits of IVF treatment?

IVF is one of the most effective methods for infertility treatment, achieving high success rates. In cases where couples are unable to conceive using their own reproductive cells, IVF also allows the use of donor cells.

How long is the treatment process for PGT-A (preimplantation genetic testing of embryos)?

After egg retrieval, fertilisation, and subsequent extended embryo cultivation to the blastocyst stage, we perform a biopsy on the embryos. The embryos are then frozen. Approximately 30 days after the sample collection, the test results are available. Once we have the results and it is confirmed that the embryo is healthy, the patient begins preparation for a frozen embryo transfer in a new cycle. The subsequent embryo transfer takes place around the 18th day of the menstrual cycle. In the case of a frozen embryo transfer, the patient only visits Prague for 2-3 days.

Is IVF treatment painful?

IVF involves several phases, and the level of discomfort or pain may vary among individuals.

Hormonal stimulation:

  • Hormonal medications used to stimulate egg production may sometimes cause mild side effects such as bloating, mood swings, and breast tenderness.

Egg retrieval:

  • It is usually performed under sedation or general anaesthesia, so the patient should not feel pain during the procedure itself. After the procedure, some women may experience mild cramping, bloating, or discomfort similar to menstrual cramps, which typically subsides within a few days.

Embryo transfer:

  • Embryo transfer is a relatively quick and straightforward procedure that usually isn't associated with significant pain. Some women may experience mild discomfort or cramping during or after the transfer.

Luteal phase support:

  • Hormonal medication like progesterone are commonly used to support the uterine lining during the luteal phase. While some women may find the injections mildly uncomfortable, others may not experience significant discomfort.
Does IVF treatment have any side effects?

Among the most common side effects of hormonal stimulation are redness at the injection site, breast tenderness, bloating, and mood swings. These symptoms typically subside within a few days after egg retrieval.

In some cases, the ovaries' response to hormonal stimulation may be excessive, resulting in ovarian hyperstimulation syndrome (OHSS). However, thanks to detailed medical tests and customised stimulation treatment for each patient, the risk of this complication at our clinic is minimal.

In addition to the individual approach that our specialists offer each patient, discipline on the part of the woman herself is important to prevent complications. Patients should responsibly follow all instructions and recommendations provided by the doctor and staff.

What infertility treatment is most effective?

Because every treated couple is unique, it's not possible to generalise which assisted reproductive method is the most effective. Each couple will reach the desired outcome—a baby—through slightly different paths.

Do you offer treatment options for single women and same-sex couples?

According to the laws in the Czech Republic, assisted reproduction methods can only be provided to heterosexual couples where the woman is under the age of 49. For this reason, we cannot treat gay and lesbian couples or singles at GENNET.

If a woman is considering delaying motherhood to a later age for any reason, we recommend considering social freezing, which allows a woman to freeze her eggs for later use.

How to best prepare the body for IVF?

Before IVF, it's not necessary to follow any specific regimen. However, we highly recommend maintaining a healthy lifestyle, including:

  • Maintaining optimal weight
  • Avoiding smoking and limiting alcohol consumption
  • Reducing caffeine intake
  • Minimising stress
  • Taking appropriate dietary supplements
Is there a BMI limit for IVF treatment?

The BMI (body mass index) value significantly affects the success of IVF treatment, which is highly reduced when it exceeds 35.

In overweight patients, the retrieval of oocytes (eggs) is usually technically challenging – the ovaries are poorly accessible for the aspiration needle, and the yield of oocytes may not be adequate. During attempts at aspiration, there is a higher risk of injury to surrounding organs (intestines, large blood vessels, or bladder) due to reduced accessibility. In the event of such an injury, urgent surgery in the operating theater is necessary, which we are unable to provide at our facility.

Administering general anaesthesia to overweight patients is also very risky.

To ensure the safest and most successful treatment, we have set a BMI limit of 35 for IVF treatment with patient's own oocytes. If a patient exceeds this limit, oocyte retrieval cannot be performed. For all other types of treatment, the BMI limit is set at 40.

Where can medication be obtained?

Your personal coordinator will recommend where to obtain medication in your home country. Prescriptions are sent to patients electronically.
In the case of a personal consultation at GENNET, it is possible to purchase medication directly in Prague. Buying medication in the Czech Republic is more cost-effective for patients and reduces unnecessary stress.

How long will I need to stay in Prague?

Most patients arrive for the second ultrasound check around the 9th day of stimulation. Egg retrieval typically occurs around the 14th day, followed by embryo transfer 5 days later. Therefore, patients will spend approximately 7-10 days in Prague, depending on the stimulation process.

Where to stay in Prague?

GENNET Clinic is located directly in the historical centre of Prague. It is easily accessible from the airport by both public transport and taxi. Thanks to partnerships with numerous hotels in close proximity to the clinic, you can take advantage of their services at a discounted price. Your coordinator will provide you with accommodation recommendations.
