What is IVF with Donor Eggs?

What is IVF with Donor Eggs?

  • A treatment method for couples with fertility disorders using in-vitro fertilisation (IVF) with donor eggs.
  • During this process, the donor’s eggs are fertilised with the patient's partner’s sperm.
  • The resulting embryos are then transferred to the patient's uterus.
  • This method is recommended for patients who, for various reasons, have been unable to conceive and carry a healthy child using the patient's own eggs.
  • In cases of genetic abnormalities on the part of the patient, IVF with donor eggs can help prevent the transmission of genetic defects from mother to child.

In-vitro fertilisation (IVF) with donor eggs is an infertility treatment method where the donor's eggs are fertilised in laboratory conditions with the patient's partner’s sperm. At GENNET, we typically fertilise the eggs using the ICSI method (part of the IVF with donor eggs treatment package), where a selected sperm is injected directly into the egg using a micro-needle. We also utilise other special laboratory methods. The embryos are then placed back into the patient's uterus during embryo transfer.

During IVF treatment with donor eggs, we stimulate the donor’s ovaries with medications commonly used for traditional IVF with the patient’s own cells. Simultaneously, we prepare the patient's uterus with medication to optimise its readiness to receive the embryo.

IVF with donor eggs offers hope for couples facing various causes of infertility, such as advanced maternal age, poor egg quality, or genetic disorders.

In order to eliminate potential health risks for the future parents and their baby, partners must undergo a series of medical tests. These include basic gynaecological examinations, semen analysis, and blood tests for both partners. Similarly, the egg donor undergoes a series of detailed medical examinations including genetic testing. Detailed medical tests help us protect the health of both the patient and the egg donor.
You can undergo these examinations either at our clinic or in your own country. If you choose the latter, your coordinator will send you all necessary documents for the health tests electronically.
GENNET's doctor will then request the test results necessary for the treatment. They may also recommend additional examinations to help improve the treatment success. You can send us the results via email, and then you will only need to come to Prague for the final part of the treatment, approximately 5-7 days.
Based on the results of all examinations, the doctor will establish a comprehensive treatment plan and recommend suitable laboratory methods to maximise the chances of successful pregnancy.

We synchronise the menstrual cycles of both the patient and the donor using contraception (or alternatively, Norethisterone). Subsequently, the donor undergoes hormonal stimulation to mature a larger quantity of eggs than in a natural cycle. Hormonal stimulation typically lasts about 14 days.
At the onset of her menstrual cycle, the patient also begins preparation for embryo transfer. Based on a tailored stimulation protocol, she takes medications aimed at optimally preparing the uterine lining for embryo implantation. During this preparation phase, the proper growth of the lining is monitored through 1-2 ultrasound examinations.

Hormonal
Preparation Hormonal Preparation

Approximately 14 days after hormonal stimulation, when the follicles have reached optimal size for retrieval, the donor undergoes egg retrieval. The procedure is conducted under ultrasound guidance using a special thin needle inserted through the vaginal vault to the ovaries. The donor is under short general anaesthesia during the procedure.
On the day of egg retrieval, the patient's partner provides a semen sample. Alternatively, previously frozen sperm samples can be used for egg fertilisation.

Egg Retrieval from the Donor and Partner Sperm Egg Retrieval from the Donor and Partner Sperm

The eggs retrieved are subsequently fertilised with the patient's partner's sperm under laboratory conditions. At GENNET, we typically use the ICSI method for egg fertilisation.

Additionally, at this stage of treatment, based on recommendations from the doctor, embryologist, or patient preferences, other laboratory methods may be employed to increase the chances of successful treatment:

  • ICSI - intracytoplasmic sperm injection directly into the egg (part of the IVF treatment package with donated eggs)
  • MFSS method - for selecting the highest quality sperm
  • Extended cultivation - method for selecting the highest quality embryos (part of the IVF treatment package with donated eggs)
  • Continuous embryo monitoring - ensures optimal conditions for embryo development with continuous monitoring
Fertilisation of Eggs and Embryo Cultivation Fertilisation of Eggs and Embryo Cultivation

After 5 days of embryo cultivation, the embryologist selects the embryo with the highest potential for successful implantation in the uterus and subsequent healthy pregnancy.
The quality of the embryo is determined by:

  • Egg quality
  • Sperm quality
  • Combination of genetic information from both partners

Due to the high effectiveness of assisted reproduction methods, treatment success, and efforts to minimise risks associated with multiple pregnancies, typically only one embryo (in exceptional cases, two embryos) is transferred into the patient's uterus.
The embryo transfer procedure is painless, performed without anaesthesia, under ultrasound guidance, and the patient can typically leave the clinic about 15 minutes after the procedure.
If multiple promising embryos are obtained during the treatment cycle, they can be preserved for future use through vitrification (freezing).
Selection of the suitable embryo for transfer can also be assisted by preimplantation genetic testing. In this case, the embryo undergoes genetic testing before transfer to the uterus. We offer three methods for such genetic testing of embryos:

Embryo Transfer
into the Patient's Uterus Embryo Transfer into the Patient's Uterus

Approximately 14 days after embryo transfer, the patient undergoes a blood test to measure hCG levels. This hormone level determines if the patient is pregnant. Within the following two to three weeks, the patient undergoes an ultrasound examination to confirm foetal heart activity. Medication is typically continued until the 12th week of pregnancy.

2. Selection of donor and synchronisation of cycles

Donation of gametes in the Czech Republic is voluntary, completely anonymous, and non-commercial (donors are compensated for expenses related to the donation process - medications and vitamins, transportation, accommodation if needed, lost wages). Patients only learn basic non-identifying characteristics about the donor. From our extensive database, we select a donor based on the physical characteristics of the couple, including blood type and phenotype (eye colour, hair colour, height, weight, skin colour, ethnicity).

All our donors must undergo a series of detailed medical examinations including genetic testing.

The doctor will plan the treatment to synchronise the menstrual cycles of the patient and the donor so that both women are prepared for egg retrieval and subsequent embryo transfer at the right time.

3. What Tests Does an Egg Donor Undergo?

At GENNET, we have strict criteria for egg donors, and each donor must undergo a series of detailed health tests.

1. Medical Screening and Gynaecological Examination

The medical screening of a donor typically includes the following tests:

  • Complete blood count,
  • Hormone tests for AMH and TSH,
  • Blood type and Rh factor,
  • Tests for infectious diseases and sexually transmitted infections (HIV, hepatitis B and C, syphilis),
  • Drug and substance abuse screening,
  • Routine gynaecological examination including ultrasound.

2. Genetic Consultation and Testing

During the genetic consultation, the potential egg donor discusses with a geneticist the history of diseases present in her family, the ethnic background of her extended family, and other significant hereditary predispositions.

This meeting is focused on genetics and provides us with another opportunity to get to know the potential donor and ensure the highest chance for patients to have a healthy baby.
As part of this genetic testing, the donor undergoes:

  • Karyotype analysis: assessment of chromosome structure and number,
  • Genetic tests for commonly occurring genetic diseases in the population:
    • Cystic fibrosis,
    • Spinal muscular atrophy,
    • Deafness,
    • Sex-linked genetic disorders.

4. Hormonal Preparation

All embryos undergo preimplantation genetic testing at the blastocyst stage (approximately 5-day-old embryos after extended cultivation). After sample collection, the embryo is frozen. The results of the testing process take about 30 days. Upon receiving the results, the patient begins preparation for the transfer of a healthy embryo in a new cycle. In the case of frozen embryo transfer, the patient typically visits Prague for only 2-3 days.

8. Pregnancy Test and Monitoring Embryo Implantation Success

What Precedes IVF Treatment with Donated Eggs?

During your initial consultation with one of our specialist, your complete medical history will be thoroughly discussed, and all previous examinations will be reviewed. To gain a comprehensive understanding of the current health status of the treated couple, a series of comprehensive tests will be conducted based on the doctor's indications. These tests aim to clarify your health condition and identify any reproductive disorders.
Recommended examinations vary among individual couples. The basic tests that all patients undergo include the following:

Examination of the woman

  • Gynaecological ultrasound examination
  • Blood tests: TSH (Thyroid Stimulating Hormone), PRL (Prolactin)

Examination of the man

  • Semen analysis: basic fertility test evaluating the quality and quantity of sperm in the ejaculate

Examination of both partners

  • Sexually transmitted diseases (STD) testing*

Frequently Asked Questions

How long does the entire process of IVF treatment with donated eggs take?

The total duration of treatment is individual for each couple and depends on many factors. The shortest possible duration is approximately three menstrual cycles of the woman. On average, from the first consultation to embryo transfer, the process takes about 3 months.

What is the age limit for IVF treatment?

According to the Czech Republic´ s law, we can provide assisted reproduction methods to heterosexual couples where the woman is under the age of 49 years (48 years and 364 days).

Do I need a referral from my gynaecologist or another doctor for my first visit to GENNET?

A visit to the assisted reproduction centre is recommended for couples who have had unprotected intercourse for at least one year without achieving pregnancy (for couples where the woman is over 35 years old, this period is shortened to six months).

A doctor's recommendation for a consultation at GENNET is not necessary. However, if you have already undergone any examinations, please bring the results with you.

What are the benefits of IVF treatment with donated eggs?

IVF treatment with donated eggs is one of the most effective methods for infertility, achieving high success rates.

Is IVF treatment with donated eggs painful?

IVF treatment with donated eggs involves several phases, and the level of discomfort or pain may vary among individuals.

  • Hormonal Stimulation:
Hormonal medication used to prepare the uterine lining for embryo implantation can sometimes cause mild side effects such as bloating, mood swings, and breast tenderness.

  • Embryo Transfer:
Embryo transfer is a relatively quick and simple procedure that usually isn't associated with pain. Some women may experience mild discomfort during or after the transfer.

  • Luteal Phase Support:
Hormonal medications like progesterone are commonly used to support pregnancy during the luteal phase. Vaginal progesterone application is often recommended as it has shown the best treatment outcomes in terms of success rates.
Does IVF treatment have any side effects?

The most common side effects of hormonal stimulation include breast tenderness, bloating, and mood swings.

To prevent complications, besides the individual approach by specialists, the patient's discipline is crucial; they should responsibly follow all instructions and recommendations from doctors and staff.

What infertility treatment is most effective?

As every treated couple is unique, it's not possible to generalise which assisted reproductive method is the most effective. Each couple can achieve the same result - a baby, through slightly different paths.

Can I choose my donor?

Donation of gametes (eggs or sperm) in the Czech Republic is completely anonymous. A doctor, together with our specialised donation team, selects a donor from our extensive database based on physical characteristics of the couple such as height, weight, eye and hair colour, ethnicity, skin colour, and blood type.

Who are your donors?

Our donors are primarily university students or young mothers aged 18-34 years. Before being included in the donation program, donors undergo a series of examinations and provide a detailed personal and family medical history. All our donors undergo gynaecological examinations, genetic testing, and are repeatedly tested for sexually transmitted diseases.

Can I meet my donor?

Egg donation in the Czech Republic is completely anonymous, which means there is no possibility of any connection between the recipient and the egg donor.

Are donors paid for donating eggs?

According to the law of the Czech Republic, egg donation is completely non-commercial. However, donors are entitled to compensation for expenses related to the donation, such as travel costs and lost wages.

Do you offer treatment for single women and same-sex couples?

According to the law of the Czech Republic, assisted reproduction methods can only be provided to heterosexual couples where the woman is up to 49 years old. For this reason, GENNET cannot provide care to gay and lesbian couples or singles.

If a woman is considering delaying motherhood for any reason, we recommend considering the option of social freezing, which allows a woman to freeze her eggs for later use.

How to best prepare the body for IVF?

Before IVF, it is not necessary to follow any special regimen. However, we highly recommend maintaining a healthy lifestyle:

  • Maintain optimal weight
  • Avoid smoking and limit alcohol consumption
  • Reduce caffeine intake
  • Minimise stress
  • Take appropriate dietary supplements
Why is egg donation better than adoption?

If both the partner's gametes and the donor's gametes are used for fertilisation, your child will inherit your genetic information from one entire half.

Together with your partner, you will experience an unforgettable period of pregnancy. During which a woman can influence the nutrition of the foetus. At the end of the journey (or rather at its beginning), a unique experience of childbirth awaits you.

What guarantee does the IVF treatment package with donated eggs offer?

The guarantee program ensures the client receives at least 2 embryos at the blastocyst stage (day 5 of cultivation). This is under the condition that sperm quality does not qualify as OAT (oligoasthenoteratozoospermia) or worse according to WHO standards. If the client receives only 1 blastocyst and does not achieve pregnancy (confirmed heart activity), at least 1 additional embryo will be provided in an asynchronous cycle (i.e., 1 embryo at the blastocyst stage using eggs from another donor).

Additionally, a third cycle is offered free of charge for clients who do not achieve pregnancy (confirmed heart activity) after two cycles with donated eggs (following the transfer of all embryos created during these cycles), and sperm quality in both cycles does not qualify as OAT or worse according to WHO criteria.

Is there a BMI limit for IVF treatment?

BMI (body mass index) significantly affects the success of IVF treatment. Elevated values increase the risk of complications during IVF treatment and subsequent pregnancy and delivery. To ensure the safest and most successful treatment, we have set a BMI limit of 40 for IVF treatment with donated oocytes.

Where can I get medications?

Where to obtain medications in your own country will be recommended by your personal coordinator and the prescriptions are sent electronically to patients.

For personal consultations at GENNET, medication can be purchased directly in Prague. Buying medication in the Czech Republic is cost-effective and stress-free for patients.

How long will I need to stay in Prague?

Most patients arrive in Prague the day before the donor's egg retrieval and leave the day after embryo transfer. In Prague, patients typically spend about 7 days depending on the course of stimulation.

Where to stay in Prague?

GENNET clinic is located directly in the historic centre of Prague. It is easily accessible from the airport by both public transport and taxi. Through partnerships with numerous hotels near the clinic, you can benefit from discounted rates. Your coordinator will provide you with tips for accommodation.
